Policy on Service Connection

1. Minimum service connection components shall be as shown in Fig. 1. Fig.1   2. Service connection components up to the swing valve as shown in Fig. 1 shall be installed outside the fence or property of the customer for easy inspection and maintenance.   3. Service connection with a total distance of 6 meters or more measured from the tapping point to the nearest property line of the customer shall be installed at a suitable place nearest the tapping point as determined by the Maintenance Foreman. 4. Except for the water meter and magnetic lock valve (which will be provided by HWD), the customer has the option to provide the materials needed for the service connection provided that such materials conform to the specifications of HWD. The materials shall be inspected prior to the approval of the application for service connection. 5. Applicants shall be responsible in securing all necessary permits and/or clearances that maybe required by other government agencies (i.e. Municipal/DPWH excavation permit, etc.). 6. The applicant or his authorized representative shall be required to attend the orientation/briefing on HWD’s policies, rules and regulations. 7. The application fee for new service connection is P2,500.00. 8. Effective on November 2, 2015.

New Reconnection Policy

SCHEDULE OF RECONNECTION FEES AND OTHER INCIDENTAL EXPENSES A.) If the approved application for reconnection is within 180 days from disconnection date: A.1.) Service connections equipped w/ magnetic lock valve (MLV) - Unpaid Water Bills (including penalty) + P50.00 Reconnection Fee. A.2.) Service connections NOT equipped w/ MLV - Unpaid Water Bills (including penalty) + P400.00 Reconnection Fee + P50.00 Inspection Fee + Cost of other materials/labor needed for the installation of MLV. B.) If the approved application for reconnection is within 181 to 365 days from disconnection date: B.1.) Service connections equipped w/ MLV - Unpaid Water Bills (including penalty) + P100.00 Reconnection Fee + P50.00 Inspection Fee. B.2.) Service connections NOT equipped w/ MLV - Unpaid Water Bills (including penalty) + P500.00 Reconnection Fee + P50.00 Inspection Fee + Cost of other materials/labor needed for the installation of MLV. C.) Service connections that remain inactive for more than 365 days from the disconnection date shall be automatically removed/disconnected at the tapping point (mainline). Recovered/salvaged service connection materials shall be turned over to the Concessionaire, except for the water meter and MLV. Reconnection of water service maybe availed subject to the following conditions: C.1.) Availability of sufficient water pressure/supply in the area. C.2.) Payment of Unpaid Water Bills (including penalty and other charges, if any). C.3.) Payment of P500.00 Meter Maintenance Fee. C.4.) Payment of P200.00 Reconnection Fee, if equipped w/ MLV and P600.00 if NOT equipped w/ MLV. C.5.) Payment of P50.00 Inspection Fee. C.6.) Payment for the cost of labor and new service connection materials. D.) All request/application of water service reconnection may incur other incidental expenses (i.e. Misc. Plumbing Materials, Excavation/Concrete Cutting Works, etc.) as reflected in the inspection report of the Maintenance Crew. E.) Ang Hermosa Water District (HWD) ay mamimigay ng buwanang papremyo sa kanyang mga konsyumer bilang isang paraan ng pagpapasalamat sa kanilang patuloy na pagtangkilik. Ang detalyadong paraan ng pagsali ay ang mga sumusunod: Ang raffle promo ay bukas sa mga kasapi/konsyumer ng HWD na aktibo at walang pagkakautang hanggang sa itinakdang araw ng pagsusumite ng entrada (entry). Apat (4) ang mananalo bawat buwan ng (1/2) kalahating kaban ng bigas bawat isa. Ang promo ay magsisimula sa buwan ng HUNYO 2015. Ang pagbola ng mananalo ay gaganapin sa opisina ng HWD tuwing ika-apat na Biyernes ng bawat buwan sa ganap na alas diyes (10:00am) ng umaga. Kung ang araw ng pagbola ay tumapat ng “holiday” o idineklarang walang pasok sa susunod na Biyernes gagawin ang pagbola. Lahat ng ineteresadong konsyumer ay maaaring saksihan ang gagawing pagbola ng mananalo. Ang mga mananalo ay ipapaskil sa HWD Bulletin Board at ipapaalam sa pamamagitan ng “text message”, tawag sa telepono o padadalhan ng sulat. Ang mga empleyado at mga “Board of Directors” na konsyumer din ng Hermosa Water District ay hindi kasama sa gagawing “raffle”. Ang HWD ay may karapatang beripikahin kung ang mga nanalo ay kwalipikado at kung ito’y napatunayan na hindi bubunot muli ng ibang entrada upang makumpleto ang apat na mananalo. Ang mga konsyumer/kasapi na maraming koneksyon ay maaaring mabunot at manalo ng maraming papremyo sa bawat raffle draw. Ang mga nanalo ng konsyumer/kasapi ng nakaraang buwan ay maari pa din manalo sa mga susunod na “raffle draw” bawat buwan. Ang mga hindi nanalong entrada (entry) sa bawat buwan ay hindi na maaring gamitin sa susunod na buwang pagbola. Bagong entrada(entries) ang bobolahin bawat buwan. Lahat ng papremyo ay hindi maaaring palitan ng pera at walang tax. Ang ano mang gastusin o pamasahe ng sino mang sumali at sa pagkuha ng premyo ay hindi sasagutin ng HWD. PARAAN NG PAGSALI: Bayaran ang inyong "water bills” o ibang bayarin sa takdang panahon. Pirmahan ang itaas ng inyong pangalan na nakasaad sa “statement of account” o berdeng resibo na ipinamimigay ng mga kolektor at isulat din ang inyong numero ng telepono o “e-mail address. Ang kasalukuyang berdeng resibo sa kasalukuyang buwan lamang ang pwedeng ihulog bilang entrada o “raffle entry”. Ihulog ang inyong entrada (raffle entry) sa mga itinalagang “drop boxes”. Sa opisina ng HWD tuwing regular na oras na Lunes- Biyernes 8:00am-5:00pm at sa Balsik Pumping Station sa araw ng “due date” lang pwedeng maghulog. Maaari rin ipadala sa itinalagang kolektor sa inyong lugar ang inyong entrada o “raffle entry”. Ang huling araw ng pagsusumite ng inyong entrada o raffle entry ay dalawang araw bago ang itinakdang pagbola (ikaapat na Biyernes ng buwan). PARAAN NG PAGKUHA NG PREMYO: Kung ikaw ay nanalo, aabisuhan o tatawagan po kayo ng HWD at magdala lamang ng valid ID (company ID, Postal ID o anumang government issued ID na may kalakip na larawan) sa aming opisina sa Magsaysay Road, Sto. Cristo, Hermosa, Bataan upang makuha ang inyong premyo. Kung wala ang nanalo at may “representative” kailangang may authorization letter na kasama ang ID ng nanalo at representative at kung hindi makukuha ang napanalunan sa loob ng tatlumpong araw (30 days) ay pawawalang bisa ng tanggapan ng HWD ang premyo.

First Raffle Draw

As our way of saying Thank you! for your continued support to us, your Hermosa Water District have given away on-half (1/2) sack of rice to four(4) lucky concessionaires who have paid their bill on time. Raffle draw will be held every 3rd friday of the month, from 2:00PM to 2:30PM at Hermosa Water District office.
